QSAC’s Achieve Program has openings for individuals with Autism Spectrum Disorder (Level 1: Requiring Support) and/or Mild Intellectual Disabilities. Operated under the NYS Office for People With Developmental Disabilities (OPWDD) Day Habilitation Without Walls, Achieve supports successful transitions into adulthood through increased independence, community engagement, and work readiness.
Participants—whom we call colleagues—build confidence while developing essential life and employment-readiness skills, including communication, socialization, self-advocacy, time management, and adaptive functioning. Programming is individualized and goal-driven, supporting each colleague as they explore personal interests, strengths, and future vocational pathways. Colleagues attend Lunes-Viernes from 9am-3pm & travel to the program.
Program Highlights:
• Work-readiness and pre-vocational skill development
• Volunteer placements and hands-on community-based learning
• Professional social skills and workplace behavior practice
• Relationship building and self-advocacy training
• Skill-building workshops and enriching outings to museums and local attractions
Documentation Needed for Enrollment:
- Application for Services/QSAC’s Day Hab
- Psychological & Social dated within 5 years
- Current annual physical (dated within 1 yr)
- Current PPD (dated within 1 year)
- Level of Care Eligibility Determinations (LCED)
- Life plan with DH addendum
- TABS inquiry (CR4)
- Day Hab Authorization
Next Steps:
- All prospective participants must complete trial visits prior to beginning the enrollment process.
- Potential participants will also need to complete an intake meeting prior to enrollment
