About the Program

The Community Habilitation (Comm Hab) program is designed to assist the person to develop or enhance their skills to live more independently at home and in their communities. Comm Hab goals are derived from the person’s Life Plan that is developed during the Person-Centered planning meeting. Comm Hab develops a Staff Action Plan, that identifies the activities used to help the person to achieve their desired goals. A Direct Support Professional (DSP) implements the activities from the Staff Action Plan. The DSP is scheduled to work 1:1 based on the agreed time and location. Services can be provided at any time in a person’s home and/or in the community. 

Goals and Supports can include:  adaptive skills, community inclusion, social skills, travel training, money management, leisure skills, appropriate behavior, personal care, supervision, protective oversight and exploration of new experiences.

Service Delivery Options:

Direct Provider Purchased (DPP)

Available with or without budget authority. QSAC acts as the Employer and Manager of Record: handles staffing, payroll, hiring, training, human resources, scheduling and monitoring, maintaining regulatory documentation and billing.

Agency Supported with Memorandum of Understanding (MOU)

Is a Self-Direction Model available with or without budget authority. QSAC acts as the Employer of Record: hires staff referred by the person, handles payroll, human resources, training and maintains regulatory documentation and billing. The person or their representative acts as the Manager of Record: refer staff who will provide the Respite service. Through a Memorandum of Understanding (MOU) the person/representative will have more control over how and when the services are provided, staff training, schedule and activities needed to support the person.

The person must meet OPWDD Eligibility requirements.
